To order your SB Performance Card, simply fill out the form to the right. There is no charge for this card and it will be posted directly to the address you supply.  If you have any questions, please contact Julie and she will assist you further.

PLEASE NOTE:

• You MUST have read and understand the information about SB Performance Cards.  If you have not done so, please read here.
• You MUST declare all wins that this horse has received either with you and/or previous owners. Please supply win, show name and show date in the following classes;
  • Novice 
  • Open 
  • Lady's Standardbred 
  • Gentleman's Standardbred 
  • Novice Paced and/or Mannered 
  • Open Paced and/or Mannered 
• Champion + Reserve Champion placings are NOT classed as "wins".
